144 Hz monitor
Okay it's July of 2019. Is there more of a fix than what's already out there? My games runs at 72 FPS which makes sense confsidering the 144 is halved. There is an article that talks about setting PC to 120 HZ and
ipresetinterval = 2 but that made the FPS 30 for whatever reason. I just bought the 2080 Ti and already had to deal with the weapon debris CTD issue. Thanks for any info!

set ipresetinterval = 0 (this disables the game's inbuilt Vsync)

In your Nvidia control panel, in the profile for Fallout 4 force the Vsync to on.

Set your desktop to 120 hz (any more than this and the game starts to speed up due to how the physics is coded)

Run the game in borderless window mode.

Enjoy your 120fps Fallout 4 (apart from the bad parts of the central city obviously).

Nope, they fixed that a long time ago. Just make sure you have all the above settings & methods set, and you're good to go.

No it's not. Not with my method above anyway. I've got like 400 hours at these settings. I did a lot of testing.

Besides, i run my monitor in ULMB mode, which doesn't function unless the FPS is a genuine 85FPS minimum. 120FPS is optimum for it.

Flippin genius! Now question.. I'm running 2080 Ti... are max settings with high res texture pack worth it? Some advice for settings would be appreciated as well, thanks!

I could have sworn going above the game-set cap screwed with the physics making everything go haywire

I personally don't think the max texture pack is worth it. Definitely not unless you're running it off an SSD anyway.

I use Reshade to sharpen the game, so i don't feel the need for the HD texture pack. You could try and see if you like it the HD textures.

I was replying to the comment that suggested it doesn't run at a genuine 120FPS.

120FPS seems to be the limit for a normal playing game. Going above that you do start to see speeded up running and screwy things.

Do you guys touch godrays or shadows? I'd love to keep the visual up, but not dip below 60 FPS, except for known areas.
There's a tweak on pcgamingwiki for high quality godrays with low performance hit, without the annoying blockiness you get on anything other than ultra too. It's awesome :)

Hey I guess I was doing it wrong, but when I went back and tried your solution again there was some crazy speed and FPS going on... any idea what happened?
I'm not sure man. With my method above, the game should be hard limited to 120FPS and no tearing as well. Are you sure you have ipresetinterval 0 set? it sometimes resets itself when you make other changes in the settings menu.

All the steps in my list above are essential to it running nicely.

Make sure it's running in borderless window mode too. As fullscreen can default to 144hz, and then the game will try to run at 144FPS. It needs to be running at 120hz/FPS.
