So survival is not hard enough for you? Then try survival using melee only weapons and minimal armor and see how you fare.

Define what you consider "difficulty."

The game already has like 5 or 6 difficulty options, so what do you want that those don't do?

For instance, past L30 the game gets increasingly easy as most enemies cap between 30 and 50. This mod make them scale to you indefinitely:

But it has little effect on <L30 characters. It's small, simple and mostly compatible.

Then there's this mod which changes a lot more stuff and isn't as compatible with other mods:

Mostly tweaks to HP and damage but it touches all NPCs and weapons.

Then there's Horizon, the least compatible of the group because it's a massive overhaul:

I use...

https://www.nexusmods.com/fallout4/mods/26976 which lets you adjust the damage that you take and the damage that you deal. Plus a bunch of other cool things.

https://www.nexusmods.com/fallout4/mods/11853 overhauls mutants and makes them more dangerous...adds Nightkin to FO4, which is awesome.

https://www.nexusmods.com/fallout4/mods/9103 overhauls Raiders and makes them more dangerous.

https://www.nexusmods.com/fallout4/mods/30308/ adds small groups of leveled enemy stalkers to the game.

Try those and see if you like them.
