I do not know if BGS originally planned to reveal this to us in FO4 DLCs, or if it was intentionally left as a mystery for now. We can have a lot of theories, but actual origins and purpose of Gunners remains unknown.
I do not expect them to be kids from Vault 75 though. It looks like Gunners are making their way inside the Commonwealth instead outside of it. Vault 75 has water and power resources, with shelter and programmable training grounds. It is located just next to a hospital and research center.
If Gunners (as a whole) spawned from Vault 75, then they are mental midgets to abandon the area.

There appears to be no centralized commander of all local forces although the one at the Gunner's HQ (Called Captain Wess) at the broadcast station appears to resemble some control all of them.
Those who are supposedly under his command are titled Commander of which there are several spread out across the Commonwealth at various locations.
Contrary to military chain of command, we occasionally see a Major but they are considered a rank lower than Captain.

Actually they have Gunner Brigadiers...that predominantly use stealth boys ....fortunetly they wear combat helmets (that have a shining light on them). But honestly watch out for the Assaultron dominators.....they are pretty much the toughest thing in the game.
I remember running into those assaultron dominators. They are nasty.
Since you mentioned brigadier, I vaguely recall running across those.

MacCready didn't come from Vault 75. He was from Little Lamplight, a settlement in the Capitol Wasteland (Washington DC) made entirely of children under the age of 16. He ran with the Gunners in the Commonwealth for a while, but left on his own.
Vault 75's protocol was to keep the kids and let the older ones out to do their thang they had been trained for, so MacCready could not have been from Vault 75.
Obviously you reach a point where there are no more children, so the last one to leave Vault 75 would have either taken the remainders, or stayed until they were old enough to fend for themselves according to the program, and left when they did.
I honestly do not understand where the hate for Preston Garvey comes from, or why people are putting the blame on him for the collapse of the Minutemen. Turn on the dialog subtitles and pay attention to what's being said.
Preston Garvey is not the leader of the Minutemen, and has not been driving others away.
One month before the player exits their cryo-pod and begins their journey (i.e. the start of the game), the incident at Quincy happened.
Since clearly none of you know what this incident is, Quincy was being attacked and called for aid. Various Minutemen regiments showed up, including the one Garvey was in, which also included the General of ALL the Minutemen. As the battle raged, some of the weaker commanders broke ranks to try to save their own asses, and at least one of them joined the Gunners (the one in power armor on the overpass overlooking Quincy). Garvey's unit was all but wiped out, and he grabbed as many survivors as he could and ran for it.
In that entire time, Garvey has been trying to find a safe place for those survivors. For 30 days, non-stop, they've been on the move. He has not had time to round up helpers and send them to the various settlements, because they've been on the run for 30 days.
Honestly, either you people are upset because Garvey's black, or you're just idiots who can't read simple game dialog. Which one is it?
I agree Garvey (and Marcy) get a bad rap (I don't have a problem with either since I don't make my home in Sanctuary and rarely get up that way. Garvey does however need to let you catch your breath before chain gunning 3 or more quests at you. Or you can just do what I do and clear out EVERY place so there are no more quest locations (I have a mod that keeps Commonwealth quests in Commonwealth (meaning no Somerville quests for clearing out ghouls in the Supermutant Bathhouse in Far Harbor).

Marcy is understandably unpleasant since her wimpy husband let a ghoul snatch away their infant and proceed to eat it.
As for Garvey hitting you left and right with Settlement quests, if you're doing an immersive, realistic playthrough, you're supposed to be finding your son first and foremost, so these others arelargely inconsequential until you need their help. You don't lose points or standing or affinity for putting them off, they're just there when you get a moment.

That's called Hubris from one point of view, Irony from another and Hypocritical from a third.
