find your folder C:/User go into My Documents/My Games find Fallout 4
Open the file Fallout4Prefs with notepad
double check that iSize H=xxx matches your resolution (1080 for 1080p) and iSize W=xxx (1920 for 1080p width)
bFull Screen=1
launch the game and double check with the launcher settings that windowed and borderless modes are unchecked, and that your resolution matches what you've put into this file, also be sure to adjust graphics settings here just in case they've been changed.
lastly, right click on the file and make sure it isn't set to Read-Only
messing with my file to re-unlock my framerate so my G-Sync worked cause this windowed mode issue for me, and this was my fix. Mileage may vary, but I hope that I can help everyone with this.
