That said, I don't think in gameplay it really matters and boils down to what you'd rather look at in third person.

Well... its usually that male has penis.. and female vagina.

There's hardly any use of the getsex commands unless it's used in the main story, or plot essential characters.
As far as gameplay goes, the only difference is a single perk, ladykiller is for men (bonus damage against females and bonuses to speech when dealing with females, and Black Widow for females (bonus damage against men as bonus speech checks when dealing with men). Black Widow is considered to be the better fo the two because there are so many more male raiders and NPCs than there are females. But these perks aren't gamechangers so... whatever.
Just play whatever you want. The sex of the protagonist isn't all that important in the grand scheme of things. I play one or the other depending on whose voice I'd rather hear for a few hours.
Titles, Insults, and certain changed dialogue is the jist of it.
