How did you determine that? When a quest consists of multiple stages, each stage is being set if it got done. You should check the current stage instead and compare it with the stage list of that quest in fallout 4 wikia.
2. You can go backwards to any stage in the quest as long as the quest is active.
Did you use any setstage commands in the previous vault quests?
Have you tried to reset the quest and do it all again?
Did you leave the cell after setting the stage to allow it to reset?
that is an optional requirement. It depends from quest to quest. It is only required if a new stage invokes environmental changes in the same cell as where you are when you enter the console command.
For me, this was caused by unknowingly salvaging the pip-boy crates near the entrance.
If you salvage those, (and the vault-suit trunk), the questline gets stuck at 700 because that is when Barstow says to equip Clem like a 'real vault dweller'.
If you dont have those pip-boys and jumpsuits around, she never says that line and the quest hangs.
