Can I join Railroad while leader of the Institute?
Desdemona keeps on asking if I have made up my mind about joining them.
She is not aware that I am now leader of the Institute but if I agree to join the Railroad I will likely get given more of their quests.

They will turn hostile if you chose to enter the railroad HQ while having X6-88 with you

I have not played the way you did but I would guess doing so would either lead you to destroying the institute or killing everyone in the railroad as well as the brotherhood but that is all a guess on my part.

I did not wait that long to join the Railroad but I suspect that until the Mass Fusion quest you can still join. As a member of the RR I was able to accept the leadership of the Institute and still travel to RR HQ and purchase stuff. I don't see why it would be any different for you.

If I remember correctly you can be a member of all the factions until the Mass Fusion quest and the game will warn you that proceeding will make one or more of the factions hostile depending upon which faction you choose to support.

Mass Fusion seems to be the branch off part. I had this problem a while that I had two critical missions for both the Railroad and the Institute and I was wondering why the Undercover quest wouldnt trigger. Turns out, my current quest for the Institute involves killing Desdemona, so the quests bugged out.

I had to kill Shaun and make the Institute my enemy for me to continue doing Railroad quests.

The Railroad Quest and Institute Quest run parallel right until the end of both. You can even bug the game out big time by Destroying the Prydwen as the Railroad, and then complete "End of the Line" for the Institute, which effectively ends the game without the possibility of the final battle for the Institute happening. (At Boston Airport) As a Result, Liberty Prime will be permanently stuck in the wreck of the Prydwen, built by the ghosts of the Brotherhood of steel.

You have to get Underground; Undercover from Desdemona before entering the Institute to run the quests parellel. IMO, this is one of the most rewarding paths to choose in-game simply because whoever wrote the Railroad quests knew how to work them in (almost) flawlessly to really create this tense feeling of Infiltrator.

Certainly more complicated a balancing act than anything Spew Vegas could manage.

You cannot be leader of the Institute while Desdemona or Father is alive.
Most likely you are just working for the Institute and can still betray them to join the Railroad if you wish.

Fallout 4 took the New Vegas route and gave you 3 factions, where once you do Mass Fusion or other related quests:
A) You join Faction 1, and kill off Faction 2 and 3
B) You join Faction 2, and kill off Faction 1 and 3 if Faction 1 ever turns hostile
C) You Join Faction 3, and kill off Faction 1 and 2
D) You join the Wildcard Faction, and only kill off Faction 1 and 2 if they're hostile, and kill off Faction 3

You would have been Leader of Institute but Undercover if you had joined RR first.

I completed Mass Fusion. The game only warned me I would become an enemy of BOS, nothing about the Railroad.

Father is dying from cancer but still alive and I am the new leader and Desdemona is still alive.

Anyway, I have joined Railroad and have to meet up with Patriot at the Institute
to discuss freeing some synths. I suspect it is time to stop this Railroad quest.

One thing I can say if you went straight to leader of the institute no Railroad or Brotherhood. Then you missed tons of content. If You play the game again it is possible to compleat almost all content and still end up with the institute as an ally but hard decisions will have to be made along with some deception against your allys.
I thought you had to destroy the Railroad before you became the leader of the Institute.

You do "Powering Up" after becoming leader then destroy the Railroad during "End of the line" quest.
