Report a translation problem
That's going to be a mod thing, which mod I have no idea.
Now the weird part in step:
Step one: i can't enter or fast travel to red-rocket and sanctuary or even home plate (i don't try the whole map), if i CTD.
Step two: i try a quick new game and reach preston just for seing if it's a problem from bethesda or what, it work.
Since it work i launch a new one and barely starting custom face's of nora and i before a CTD.
Step three: i can't launch the game anymore it CTD for nothing ...
The only way to get back my 4days save it's to disable ALL MODS, not one but ALL ... fu.. me ...
Ofcourse it has to be a mod thing. The real question is, what mod, and how is cell loading involved.
Whenever you move to a different cell, the game engine tries to load everything in that cell while at the same time deallocating resources from the previous cell. If you look at your HDD activity light while you move, you'll see what I mean. I'm betting this is the point where you CTD.
You might be able to reduce the or even eliminate the problem by uninstalling any mod that adds HD textures or at least replacing them with low res ones if the author made them available. As to what "mod" it is, you're gonna have to run this down yourself but honestly, I think it's gonna be a combination of mods and marginal hardware.
Honestly, I hope I'm wrong and it turns out to be something easy.

RE: SuperSledge. I do not believe it is a RAM issue. My specs are:
intel i5 6600K
16GB of DDR4 RAM
Windows 10 Pro 64 bit
2x GTX980 (SLI disabled)
A few things I have tried:
- verifying steam cache and reinstalling mods
- clearing out old saves from save folder
- disabling mods in groups (settlement mods, texture mods, weapon mods etc)
- messing about with graphics options (full screen, windowless border, with and without v-sync etc)
- enabling papyrus logs and having a read through those (I wasn't able to find anything useful)
Have you managed to find a solution on your end?
The problem turned out to be the grass. It isn't performance related (I hit solid 60fps without exception - specs listed above).
If you go into the performance tab and turn grass density down to 1 and grass draw distance down to 1000, that should stop the crashing. I've been playing for about 3 hours now with no crashes, while before it was crashing every 5 minutes. I hope this helps someone.
Straight after posting the message above, I got a crash. Back to the drawing board...
I never bought the RAM because when monitoring my RAM usage during gameplay, I never exceeded 5 gigs of use.
Sad to hear Jack can't profit from his own suggestion, yet I still find it important for any future readers to strongy suggest attemping Jack's idea above, which I will also edit into the origina question.

It's the only thing that worked for me.
I have no mods installed and get the exact same thing. I have 16 gigs of ram, an i9 8 core 5ghz, 2080 ti. It always happens outdoors, never inside a building or during a loading screen and appears to be random as I can go 4 hours without having one and other times I'll have 2 in a row within 5 minutes...
I just tried lowering the grass desnity and view distance Iam praying this solves it like it did for the op.
That said, if you're ctd and you have zero mods? I'd head over to the nexus website and pick up at the very least The Unofficial Fallout 4 Patch. It should help fix a lot of issues you're bound to run into with out altering your game in any way. You do need the DLC's for it to function, but it does as intended. Fix the bugs.
