Are you actually using the chems, or is it just randomly happening without you doing anything?
Oddly, I think the solution ended up being getting to Parched status at normal speed out of combat, and then back up to Hydrated before going back into combat. That seems to be the point it fixed itself.
I would like to share the probable cause.
This seems to be an old bug. It occur in Vanilla with or without UFO4P.
While in combat humanoid companions can use chems in inventory which can bring all sorts of positive and negative effects in the player (Sucks in survive mode).
In this video Piper use chems like berry mentats and affects Sole Survivor.

I remember i put a PA mod in companion armor (highlight living targets) and were able to use it on my own.
simply dont give chems to your companion?
