Choose different paths in the main story and DLCs, build different settlements at different locations...you name it.

It says volumes. It says the industry is producing garbage mostly. It says this game is most likely the best game available at the moment, a game from 2015. It says even more.
Also, the ranting about "paid mods" concerns the Creation Club, a platform where you can buy content from a store using pre-paid credits and microtransactions in-game. These are entire optional, and mostly reskins of items already present in the base game.
The actual DLC packages, such as Far Harbor or Nuka World are well worth it.
It is also worth bearing in mind, that sales to Bethesda for the game and it's official DLC sends a clear message of the products that Bethesda should be offering us.
So buy that game and the DLC. The Far Harbor and Nuka World DLC does add more worth-while content, albeit lacking in fallout RPG-ness, but you can use mods to help you tailor the game to how you want it.

Reverb and Ambiance overhaul creates sound effects for not only ballistic characteristics of weapon fire, but also adds a richer element to the environmental sound too.
There are others that do this, but when you fire off a Laser Rifle with this mod installed.. you will never want to look back :)

Due to the internal disagreements of our current community over CC, it is best to ignore the reviews for the moment.
Get the season pass if money is not a concern. Far harbor, Nuka, and I would add automatron are great dlc's IMHO.
There are also amazing settlement mods on nexus if thats your thing.
When your educated about CC make up your own mind and vote with your wallet, dont listen to us jabber in like idiots :)
and to answer you question, yes. its worth playing again especialöly if you want to mod. as there are litteral ♥♥♥♥♥♥♥ of mods available now.
and for the DLC's well thats up to you, but i would atleast reccomend far harbour. thats a very good DLC to be honest.
I'd also suggest you take a look at my overhaul mod "Lunar Fallout" on the Nexus if you want to change the feel of the game. Even if you don't use it, there is a list of recommended mods like the above mentioned Reverb and Ambiance on the mod page.
There are also other overhaul mods that are well done and change the balance of the game significantly.

I suggest playing on survival difficulty. This setting completely changes the "feel" of playing the game. You are forced to be aware of your surroundings constantly, even in places you've already traveled through. Perks that IMO are marginal become far more powerful, namely many of the endurance perks such as lead belly aND ghoulish. Unlike hard and very hard, on survival both your weapons and those of NPCs do much more damage. The fights feel much more like a desperate life or death affair than on the other settings.
