Some people have said this is one of the things that may happen if the frame rate is to far above 60fps.
Yes i saw that too but... Use console dont disable Achievements?

If you're worried about achievements, the old wisdom in here was that the console commands would prevent further achievements "for the current play session". All you had to do was log off and log back in again, that made it a new session. Maybe that's changed, maybe it was never true? I don't really know.
Are there a Unofficial Patch ? If use this Patch need use other mod for enable the Achievments?.
Why Bethesda dont fix this bugs ? i cant understand. Bethesda use Creations Club for shell free mods and cant fix bugs ?
For this policy I have put negative review to the game, by Creations Club and stupids Bugs. Because this bugs looks like Early Access games or Day 1 launch but not for 2 years, incredible.
Fallout 3 is UNPLAYABLE, need install a lot of mods and unofficial patches. Fallout New Vegas need unofficial patches too. Fallout 4 too ? come on Bethesda fix your games.
Just enable v-sync and you are good to go.
This bug is fatal for people playing on survival, who cannot save that often.
