Not trying to be a jerk or troll or anything; if you are running into a 'maximum' that the GPU can put out, it may be the max calculations/rendering the thing is capable of (playing video of the menu and loading screens is a different type of calculations/rendering).
It could also be a sign that your GPU is having issues (a power or bus problem, etc). Might want to run some tests on it or at least Benchmarks to see if it happens doing anything else. Is it only 3D rendering? Is it only Fallout 4?

Oh, I see. Sorry, I was assuming that could possibly be the GPU's full potential. But since you run on very low settings and it is exactly the same...
It must be 'getting told to stay' at that framerate somewhere.. (unless it is a hardware issue, but we'll assume it is not one for now and that it is Software/Configuration somewhere)
You said it was an AMD GPU?
In Crimson, there is a FRAMERATE TARGET CONTROL in the Global Settings under the Gaming Tab. Is that 'Off'? I had mine set to 120fps usually, but for some games I had to turn it off.
Each game also has a Profile under the Gaming Tab. Check that Fallout 4's FTC is also turned off...
That's my idea for now. Might throw more in the pot if nothing changes, if that's alright.
I haven't had troubles with it, but of course it could cause some people problems, everyone's system is different.
(I just tested it with my old GPU, an R9 280 that I gave away when I got my 1060 for Xmas, today - and it seemed to work ok. It restricted a game from 180+fps to 60fps.)
