If you talk to mama murphy in concord before the attack or progress in the main story long enough he'll get renamed to his proper name.
BUT... this isn’t really recommended for major NPCs like companions as it can lead to problems (or at least that’s my understanding).
1. Under Miscellaneous select Message and then double click on the BlankMessage in the field to the right.
2. Change the ID to what you want the name to be with no spaces (e.g. DarthVader) and then change the Title to make the name look the way you want it (e.g. Darth Vader) then click “OK”.
3. A box will pop up asking you if you want to Create a New Form – click yes.
4. You’ve just created a new Message with a new ID number; if you scroll down in the field to right you’ll see it (e.g. DarthVader).
5. Close the Creation Kit and you’ll be asked if you want to save the new plugin file – click yes and name it or rename it in your Fallout 4 data file (where it now resides). You can go back and add more names later by using the Creation Kit to open the Fallout4.esm and your new .esp file (e.g. Vader_Names.esp or whatever) and making the renaming plugin the active file.
To rename an NPC in the game (and I would ONLY rename your settlers…) look at the NPC in question and go into the console. To find the name you want (in this case Darth Vader) type “help vader 4” without the quotes and hit press enter – it should pop right up; you’ll see the name (message) and its eight digit, numerical ID number.
Select the NPC (again, I'd only do settlers…) by clicking on them with the cursor (you should see an ID number pop up in front of them). Now type setactorfullname, space and then the ID number of the name you want to use (so “setactorfullname 00000000” without the quotes and the name’s ID number in place of the zeros) and press “enter”.
The settler in question will now be named Darth Vader (or whatever you named them) in-game whenever you’re close enough for their name to pop up, when you go into their inventory or in the subtitles when they speak.
Cheers!
