My first thought is that when you re-downloaded the game you also downloaded the free HD Texture pack DLC. If you did then that is quite possibly why your performance is down. Check your install size for the game. Without the texture pack it should be about 30GB, with it about 85GB. If you did download it, uncheck the box in you DLC for the game then restart Steam. That should uninstall the Texture pack and get your performance back to where it used to be.

Edit; To check your install size right click on the game in your library, properties then local files.

Try verifying the integrity of the game files. Right click on the game, properties, local files then verify.

Also, try going into the settings menu and disable controller support[won't help if you play with a controller]. For some players with constant stutter disabling it helps a lot.

Sorry, it's the game settings menu. I should have posted it that way.

You're welcome.

Also, try lowering Shadow Distance to medium or low and turn Godrays off.

No, not yet.

First try going to the game settings then Gameplay. Use the slider to raise your Look Sensitivity.

If that doesn't help, go to Documents/My Games/Fallout 4 and delete the Fallout4.ini and the Fallout4Prefs.ini. Then verify the game files again and launch the game. That will create new .ini files and might fix the problem.

You could also try going to you pc's control panel, mouse properties and increase mouse acceleration[Motion speed] a SMALL amount at a time to see if that helps.

So strange tried what you said above it was running perfect before all on ultra 72fps now it's only 30fps. I haven't done anything different. All my other games are playing the same fps they normally would.

Anyone else had this problem?

Disable Vsync in the Nvidia control panel if on.

or try playing in Borderless windowed mode.

Hi there. I had the issue just few days ago. Have been playing almost 2 years, with no worries at all. Suddenly menu screen mouse lag. I went on searching fix and tried all vsync tips from nvidia control panel etc. When i enabled vsync the mouse movement was smoother, but screen started to tear in movement. When I took vsync off, tearing stopped, but mouse lagg returned. I installed all kind of crap to try to fix the problem.
Then I remembered this latest windows 10 upgrade few days ago that took very long to upload and install. I searched from web how to go back to pre updated win10 version and that fixed all issues. Again Fo4 is accurate, no lagg, no screentear. I think I never allow win to be updated again (well all else besides security updates).
This might help some of you struggling with the issue.
