Edit; To check your install size right click on the game in your library, properties then local files.
Also, try going into the settings menu and disable controller support[won't help if you play with a controller]. For some players with constant stutter disabling it helps a lot.

Also, try lowering Shadow Distance to medium or low and turn Godrays off.

First try going to the game settings then Gameplay. Use the slider to raise your Look Sensitivity.
If that doesn't help, go to Documents/My Games/Fallout 4 and delete the Fallout4.ini and the Fallout4Prefs.ini. Then verify the game files again and launch the game. That will create new .ini files and might fix the problem.
You could also try going to you pc's control panel, mouse properties and increase mouse acceleration[Motion speed] a SMALL amount at a time to see if that helps.
or try playing in Borderless windowed mode.
Then I remembered this latest windows 10 upgrade few days ago that took very long to upload and install. I searched from web how to go back to pre updated win10 version and that fixed all issues. Again Fo4 is accurate, no lagg, no screentear. I think I never allow win to be updated again (well all else besides security updates).
This might help some of you struggling with the issue.
