UWPU - http://www.nexusmods.com/fallout4/mods/16666/? (don't mind the warning; I've tested with DSR and it's mostly fine)
DEF_UI - http://www.nexusmods.com/fallout4/mods/10654/? (optional, load after UWPU, fixes some remaining annoyances)
edit: google "how to play fallout 4 on triple screens". I know it's not the same problem; however, the same method routes you to the solution.

Thanks for the response. I think it's really bad that a AAA games developer does not provide support for these monitors considering that most of the Indie games I've been playing recently can support it (They are Billions for one).
EDIT: I've gotten it to start by using GeForce Experience and disabling my 2nd monitor, but the game runs in the 40's which is pretty unacceptable for my level of hardware. Any ideas?
