I used a combination of mods to try and get the Misery feel. The Pilgrim graphics plugin makes everything look a lot more gritty. Reverb sound mod for ambience. And better locational damage makes firefights much more intense as everyone can die in a few shots so you get those intense shootouts. Then disable the crosshair, set the HUD to almost transparent so you only see it when you look for it, and of course, play in survival mode.

Oh and I forgot but definitely a darker nights mod or something like that.
