If so try deactivating it. Could be a duplicate reference thing.
If I know Bethesda games though, it sounds like the script is bugging out when you talk to Maxson... like he's not on his proper "trigger spot", or the game still considers him to be in combat somehow, even if he's not. There could also be something else happening in the world which should be flagged as "done" but isn't... check that all hostiles in the area are dead & that none of the brotherhood are still in combat.
Does Maxson's dialogue play through properly?
After you've planted the charge & before you talk to Maxson, does the quest marker point to Maxson or still to the reactor?
If there is no quest marker, look through your quest list & make sure the appropriate quest is selected as active. Sometimes quests can deselect themselves for no apparent reason.
Off the top of my head there are some other things you can try...
1) Before you plant the charge, leave the cell and re-enter it. You might have to backtrack a fair way before you find another cell to transition to. Any cell will do. Then go back to the reactor, plant the charge & proceed as normal.
If that doesn't work, try...
2) leaving the cell & re-entering it IN BETWEEN planting the charge & talking to Maxson.
If that doesn't work, try...
3) Plant the charge, talk to Maxson and when he asks "Are you ready to teleport", answer "No".
Then walk a little way away from Maxson (out of his dialogue detection range), then return to him & start the dialogue again, this time answering "Yes".
If that doesn't work try...
4) As #3 but instead of just taking a few steps away from Maxson, leave & re-enter the cell, then talk to him again & answer "Yes".
If that doesn't work...
Phew!...
Plant the charge, shoot Maxson / Sprint at him / anything you can do to make him move slightly, then talk to him & answer "Yes".
NB:
As far as I can tell, Maxson should be standing on the catwalk near the reactor when you speak to him. If he's still walking, let him get there before speaking to him. If he's standing anywhere else & not moving, try to bump him into moving. Hopefully his pathfinding will reset & he'll then go to his trigger spot.
I play on PC so can't guarantee any of this will work - could be a platform specific bug.
Hope one of these works for you. If not I'll have a think & try to come up with some more ideas.
TL:DR - some out-loud thinking on what's causing the problem, couple more suggestions to try, possibility of using a smart phone as a keyboard to access the command console.
Long version...
So after checking the whole place you still see & hear the synths through the teleportation effect / white overlay thing?
IE the synths you're seeing / hearing are the ones you fight outside the institute when Prime makes the entrance for you - not ones that are left over inside when you teleport back to Proctor Ingram?
That's weird.
The synths outside do seem to continue spawning even after Prime has made the hole - how long they continue spawing for I don't know. Could be infinite until the quest is maked as complete. Weird because I'd expect then not to spawn at all while the game "thought" you were inside the institute.
Since you're teleporting to the hole though... that's pretty irregular. Even if the teleport location markers had been accidentally screwed up by 3rd party mods / DLC or a broken update it wouldn't make much sense because the location you're supposed to teleport to at that time is an interior cell, not an exterior one and markers generally only get mixed up like that when they're in the same cell and a mod is used that affects that cell.
Also I can't picture how any settlement mods might have interfered with anything that close to the institute - though I suppose it's not impoosible.
When you say you can't teleport back into the institute to complete the quest, do you mean you can't fast travel into it (using the teleport modification they make to your pip-boy on your first visit) or do you mean you can't re-enter the hole that Prime made? Have you tried both?
Far as I know the fast travel method stops working after you start the Mass Fusion quest for the BOS, as that's when you become permanent enemies with the Institute. Would make sense if that didn't work for you, though I'm not sure why entering through the hole wouldn't work - unless it's a 1-time scripted event & not an actual door marker. Someone who has the construction kit might be able to confirm that.
What about any other locations? Are you able to enter & exit other cells ok after teleporting back to the hole? Is anything else broken at that point?
There may be some other tell-tale signs to hint at what's going on...
If the quest is hung you may find that the rest of the world is completely lifeless - no NPC's anywhere except the quest location, doors don't work / can't enter other cells
If you're really REALLY stuck and none of this fixes the problem, I'm reading that it's possible to use a smart phone as a keyboard for the PS4. If that's indeed possible and you can figure out how to rig it up you may be able to use console commands after all.
