Basically what it's supposed to do is to change NTFS' behaviour to use more system RAM for file caching. Entered into a command shell: fsutil behavior set memoryusage 2
Then there are a couple if .ini parameters for the game you can try:
Fallout4.ini
[General]
uExterior Cell Buffer=100 (default is 47)
iPreloadSizeLimit=838860800 (default is half of this)
You'll have to play around with it and see what works. I haven't had the time yet to fully explore those. You might want to report back on those...
since your on 32GB you should be able to play longer

Man, how the hell does Bethesda get away with ♥♥♥♥♥♥ programming so easily. I need to change jobs lol
And OSes paging out memory isn't Bethesda's doing. It's part of every modern OS. By turning it off do you limit yourself and will have to restart your game more often. You can restart your game more often with or without it. Without it will you have no other choice.
I don't have the HD texture pack installed.
I know how page files, RAM, stack, and heaps work. Without writing a 3 page response: trust me, this is Bethesda's fault. This issue has been around since Skyrim.
EDIT: Forgot to mention. The reason I'm not going to disable Windows' page file can be Googled. Long story short is you should never disable your page file.
Read up on pagefiles, RAM, heap, and stacks, you'll understand why.

Then surely you can solve your own problem. Do post the fix here so you can help all of us.
That would require me to revese engineer the game, split the engine part out, find how it is perrforming memory allocation and caching files from the drive (which is probably a mess because they likely built on top of the engine since Morrowind), recompile it, test it and likely find new bugs, repeat the process until it works, and finally either get hired or sued by Bethesda.

Log your HDD activity next time you load up FO4 and you'll see what I mean.
For me (with 16 GB) without the pack it averages around 6 GB (depends on the area, 5 for Sanctuary Hills, up to 8 in downtown). Didn't notice much lagging. Then again I'm not overly sensitive to it.

The thing is it runs at 60+ FPS normally but I'll get lag spikes outside (indoors seems to load everything) as soon as the game tries to load things; my HDD being hammered during these spikes makes me suspcious about how the loading is being done.
I thought it was mods at first so I disabled most of them but the issue was sill there. Gonna try disabling everything and seeing what happens.
