You do in some places, the main map city areas and a few other places are heavy on framerate because of the Godrays and Shadow Distance, as others have discovered in other threads this game is also more memory performance demanding, not just GPU/CPU like most games
I don't see those as insignificant, what that mod is doing is removing the Tessellation effect of DX11

The mod lowers shadow distance if FPS drops lower than the targetet FPS.
Rememer to set the target FPS to 60 in the mod .ini file.
It is strange though because last year when I played it was fine on or above 60fps but now since the last up I get that horrible stuttering.
Shadow Boost - dynamically adjusts shadow distance to keep fp/s up.
Optimized Vanilla Textures (OVT) - reduces texture size and thus memory usage with little to no loss in visual quality.
Faraway Area Reform (FAR) - produces (subjectively) better looking distant objects while reducing texture memory.
Bilago's Fallout 4 Launcher - allows to tweak many game parameters in detail and can start the game with a higher task priority.
Also start with using GeForce Experience and let it optimize the game settings for you.
Disable unnecessary Windows services (see here for example: http://www.blackviper.com/service-configurations/black-vipers-windows-10-service-configurations/)
Remove any unnecessary start and background software (press Windows-Key+R and enter "msconfig" and see the tab "Services" for all the little things that get started).
Set your PC's power management to High Performance.
