Odd by any chance a mod that rebalance weapons?
Come to think of it, I believe I had the same problem back when the game was released. Memory is a little fuzzy though, so I'm not entirely sure.
Not every super mutant is the same. They can appear half naked but still can have higher resistances than one in armor, which can throw one off. Best is to look at their resistances through VATS if you have skilled the perk.
Nor is a head shot the same as a body shot. Mirelurks' softest spot are their faces (...). Chances are you are aiming at their head, but the shot lands on their chest/body, or vice versa. The centre of the cross-hair is also not always the point of impact. Input lag, triple-buffering or pre-rendering can take further influence here.
Sneak bonuses only apply as long as it says [Hidden] or [Caution], but once it says [Danger] do you not get the bonus.
Not saying you don't have a bug, but it can be many things why you are seeing this and it may not be a bug at all. I haven't noticed anything like you're seeing other than there often being further reasons behind it. If it keeps troubling you then wait until you've levelled up and switch to combat rifles for sniping. Perhaps the problem will disappear.
