Not very often.
If you're never using VATS, you're still avoiding part of the game (they didn't include all those VATS game mechanics for nothing).
As for aiming, I can aim decently, but I still can't as reliably shoot half a dozen charging ghouls manually as I can with VATS, or make sure I don't accidentally hit a suicider super mutant in his left arm as he runs to and fro around obstacles on his approach (and I don't like missing out on getting that mini nuke).
It provides a sort of balletic slow-mo instead of choppy cinematics with awkward shot queuing.
and with VATS the game would be TOO LONG , there are too many fights ...
in action mode I finish a fight in some seconds, but in VATS I would need minutes ...
and I play only in first person view !
IMO I would only use it for convenience in close range, especially melee or if an enemy got the drop on you and is closing fast. Vats is pretty useless for ranged encounters, which is where you could ads them much more efficiently. Dropping a bunch of close enemies at once, a la grim reaper sprint, is pretty cool though
Then I ran into a raider in power armor and said 'screw it, aim for the fusion core.' It didn't work.
