This bug comes from fighting Radscorpions....it is an effect that is supposed to be temporary from their poison, but sometimes it bugs out. The only fix is to reload from a previous save. Check your stats after every fight with radscorpions.

No it doesn't. If your perception gets damage from fighting Radscorpions then you have some kind of problem with your game or mods.

Due to the lack fo details im guessing you must be new to game and have not learned the mechanics.

You can use a stimpacks to heal crippled limbs and yes also heads.

Alternatively you could visit one of the many doctors located throughout the Commonwealth and pay them to heal you and yes heal crippled limbs and yes that includes heads.

^This is correct. It's also unbelieveable how people don't get mechanics, even though it does tell you whenever you get a limb crippled.

Say whatever you want, you still wrong. I tried to re-cripple my head and heal with stimpak but ofc nothing happened

There are a couple solutions. The easiest and safest is to restore a saved game from before this happened. As long as you're not playing survival, you should have several recent autosaves and quicksaves.

If your head was permanently crippled after being shot in the head, try getting shot in the head again. For some people, this fixes the problem. If you were fighting Radscorpions and got poisoned, this won't help. Instead, try putting on poison resistance equipment (for example: "Poisoner's combat armor chest piece"), getting poisoned by a Radscorpion, and quickly taking the poison resistance equipment off before the poison effect expires.

If that doesn't work and you've got access to the console, you can use player.modav to offset the Perception penalty by adding points to your Perception, as people said above. Before you mess around with the console, make sure you have a manual save you can restore in case something goes wrong.

If you're playing on survival (or don't have access to the console because you're on the Xbox or something), you can use this mod.[www.nexusmods.com] It could potentially mess up your character's stats if you don't follow the instructions, so read them.

Another option is to follow the advice in this video[www.nexusmods.com] to use a cheat program. It's a poorly made video that should have been edited down to 1 or 2 minutes, but it should work. It's a bit more complicated than simply using modav, but it's probably the only way to actually fix the bug rather than use workarounds, such as increasing your Perception.

Or you could say "Screw it!" and start a new character, like I did.

You're replying to Zulu 4 YEARS too late.
Also your inability to replicate a known fix in one attempt doesn't make Zulu wrong.

However, i can't reload my save ive been playing with 1 perception since like 180 hours without noticing it. Well if the armour does not work ill let it go

Doing it once, correctly should be enough.

You've established whether it's a head injury or venom that's the cause right?

Im not sure. This problem happened like 180 hours ago. I just did not noticed
Well if the head injury solution doesn't work, and you don't remember if it was a head injury or an insect poison, try the insect poison solution.

I removed my armor (Poisoner armor on right hand) after poisoned by bloatflies and big mosquoitoos but no success yet. Perhaps ill need a radscorp for that

Anything that fixes limbs, fixes the head limb... so says Bethesda.

You can be wrong a hundred times. It happens.
TL:DR for using Cheat Engine if you can't do the save rollback or gear swap methods:

- Install CheatEngine (decline the adware), then File -> Open Process -> Fallout 4
- Set the Value Type from 4 Bytes to Float
- Go back to Fallout, open the console and type in the following:

player.cast 6D2E2 player.right

- You are now affected by the poison and your screen should blur
- Put this into the console:

player.getvalueinfo 2C3

- Look for the number next to Modifiers: Temp:
- Put this number into the Value: field in CheatEngine and click First Scan / New Scan
- Go back to Fallout 4, close the console, and let the poison effect expire
- Reopen the console and press the up arrow to rerun the command

player.getvalueinfo 2C3

- Note the new value next to Temp: and enter that new value into the Value: field in CheatEngine, then press Next Scan
- You should now have a single entry in the address table to the left
- Right click that entry and Change Value of selected address to 0.00
- Save your game, THEN PRESS ALT+F4 TO QUIT

(If you exit through the menu, it applies a different bug to the new save... you can't aim down weapon sights, as if you were in a conversation. The video linked above missed this step because the game crashed instead of exiting normally while it was filmed.)
