Anyway, if you have been softbanned it will tell you when you start the game. Otherwise, try using White Circlet ingame.
Change your game to english, if you got banned the message won't show up unless the game is in english.
Yes, you need to delete your save file and then create a new character, wait around two to four weeks for them to review your account and hopefully remove the ban.

Those sources contradict each other for players who have already been penalized.
It's been long known that you can escape a ban by deleting your saves during the warning period. The ingame startup message tells you as much.
But as the second, official link clearly states once restrictions are placed on an account they are permanent and will not be further looked into until the player contacts support. And even then it is highly unlikely the server team will agree to lift the ban.
The only instances I can see them oblige to an unban request would be if they only detected edited weapons in your inventory or an abnormal soul increase, as these can or could be inflicted to you by other players.
But you probably need to provide a good reason why you didn't follow the instructions of the initial warning message, like if it didn't show or you didn't see it for whatever reason. Also not having been manually reported by other players too much (above average) probably helps here.
