If you get something similar, try renaming or removing libstdc++.so.6 file in Black Mesa/bin/ folder.

Interesting. Did the heads start to glitch at some point or were they always like that?
Also could you report your computer specs, driver version and distro used? This way we can see if the problem is hardware related.

Yes i did Verify Cache and all was ok, All other Source Engine games run without problems (L4D2, CS-GO, CS-Source, Etc), And other games are running without problems.
Got a bit futher this time around, made it to the test chamber then it crashed back to desktop.
Also noticed that on multiplayer server. Not sure when exactly did occur.
I played on Arch Linux x64 on radeon hd 8750m with mesa 12.0.3

Out of memory.
I'm tempted to re-install the game on my 500GB SSD drive instead of my 11TB raid. That has caused problems for some games in the past.
Problem #1 Resolved: Moved game to smaller partition fixed the memory errors.
I crash on the tram intro, without fail, when the tram passes the helicopter.
Let me know which details I should give you.

EDIT: Running Arch Linux, x86_64, NVIDIA proprietary drivers, version 375.10
This is a recurring issue with Linux gaming if you're running open-source drivers. What happens is your drivers were made with a newer version of libstdc++.so and the game is trying to use an older version, causing a conflict. Deleting the game's libstdc++.so fixes it, as you said.
I have had this problem with Steam itself, and with HuniePop from GOG.
