Laser Immunization
Do attacks from controllers or vortigaunts count as "lasers" I have to avoid in order to get this achievement?
Or is it just lasers that come from the environment (for example, in the sections where you have to ride those barrels filled with alien grunts)?

Yeah, I'm getting this thing where it won't let me get the "Black Mesa Decathlon Winner" achievement either, in spite of completing that whole section with not even a scratch on me, from Gargantuas or otherwise.
I watched a youtube video of someone getting it, couldn't spot any difference between his attempt and mine, and got sad.
Yeah, the Decathlon is one of... 4-5 achievements that didn't unlock on my first play, so.. just beat the game and replay the needed episodes. :)

You cant Quicksave/Hardsave once you enter the map (comming from the vorti-village).
You need to run through it in one go. Just make a Hardsave before you enter the loading zone for the Map and it should pop when you make the final jump across the open Gap

I followed many video guides and steam guides on this. Completed this challenge multiple times without touching lasers and the achievement was still not awarded.

It's like using manual saves or any saves invalidates the achievement or something. Is there any kind of visual thing I can look at in console to determine if and when the achievement is triggered as failed?

Try to begin a game on the level previous to the one you have to get this achievement on, or atleast go back far enough when loading a save that the level won't have started yet, and try to do as much of it as you think you can without getting hit by lasers without saving or loading, but when you do save or load, do it manually.
I mean, press esc, save game, load game, that sort of thing. When I did it I used those kind of saves instead of quick saves, and that might have something to do with it since I got the achievement.
I did that. I only used manual saves.

For me was noclip the solution xD
Okay, so I figured it out.

Manual saving is okay, but if you touch a laser at any point, DO NOT LOAD A SAVE.
You must completely quit out the entire game, re-open it, then load your save.
Only thing you have to remember is to start from the previous map, and play the map in one sitting. I loaded my save after beating Gonarch. Then just played through Interloper. You can save and load, even quick save and load when hit by a laser.
Second run through the level making sure I didn't touch any of those lasers (I checked my suit armor and health before and after each laser-event). No saves at all this time. I had to re-load the autosaves a couple of times, but still didn't get the Achievement. I didn't quit the game before loading the autosaves, though. If that's the problem, though, it's certainly not an excuse. I put a lot of hours into that Achievement and still didn't get it.

Does anyone know the map ID for just before the laser stuff starts? Maybe I can use the console to load that map and, once I recuperate, try once again. I might just wait until I finish the whole game instead of just the chapter, though.

Thanks for that link. That's a very useful Guide. Unfortunately, along with all the other "no-nos" I've found for this Achievement, loading a specific map is also specified. So, since I've now finished the game and all the other Achievements, I guess it's time to start a new game and load the Interloper chapter. I'll see what happens this time. [sigh]
Having finished the game, starting a new game and choosing Interloper and playing through without saving (only restoring to Autosaves) got me the Achievement. I didn't have to do anything else.
