Then I don't know. Plenty of bug reports and issues for lots of achievements, but nothing concrete, and the supposed experts who've been here since 1970 have nothing useful to contribute. You can remove the gamestate file from the BM folder, which will reset and rebuild any achievements you were working on, but that's not a surefire way to fix it, either.

I just replayed interloper and managed to get it on the first replay. I'll try and help outline what I did and did not do for others.
I was hit by flying objects from controllers and still got the achievement. I avoided all green fire and red electricity on platforms. I was ready to quickload in case I did hit any lasers but I either got really lucky or I learned from my mistakes on the first go around and didn't hit one single red laser on the entire playthrough. When you first have to hop out of the vent and then over a red laser, i jumped to the left towards the open area instead of to the right alongside the wall and then tossed a grenade into the cage to blow up the pulsating green sac. I remember my first playthrough I got as close to it as possible and just shot it but I may have gotten hit by something when it blew up.
EDIT . . . The achievement popped up for me in the giant room with six or nine jump pads in a circle after I hopped on one.
Also great tip about the red electricity platforms, if they count, that's where it went wrong I guess.
Did a lot of effort evading every of the obvious red lasers I came across, and was quite sure I didn't hit any of them. Welp, have fun with HL:Alyx. It's almost like how it was 16 years ago.
I got both "Laser Immunization" and "PhD in Pacifism" on my first go while quick-saving and manual saving all over the place.
After several days of trying (7-8 attempts), I finally managed to get it. Here's my tips.
1. You can load from map bm_c4a3b1 - the nearest place before the first laser. No need to start from the beginning of Interloper and go through the
2. Save/load just doesn't work - forget about saves! No matter, manual, auto or quick - if you hit the laser once - you go start from the beginning of bm_c4a3b1. I think, there's some bug that makes the game remember you got touched by the laser even if you loaded from the safe place. So if you can't get the achievement with saves, just don't use it, it'll be impossible to get it another way, at least before the patch.
3. Before the lucky try I also deleted all saves, reset an achievement in console (achievement_reset BMS_LASER_IMMUNIZATION) and deleted file gamestate.txt from C:\Program Files (x86)\Steam\steamapps\common\Black Mesa\bms.
4. If you succeed, you'll get your "reward" in the giant room with many jump pads before the elevator to Nihilanth. So, there's no need to proceed to the end of Interloper, if you didn't get the acvhievement at that point - start again.
Hope my tips could help you guys!
I've attempted several times to got this one.
I've carefully avoid lasers, and did not get the achievement.
But I tried one run without using quicksave once, just manual save and autosave instead, by starting at the beginning of the chapter, and I finally got it.
The achievement came after the 3 rooms with laser holes.
Just after you do the final jump to get out of the factory (where there is many boxes jumping at once).
