I think you are sent back to the last place you were docked at. If you are still in the tutorial area then you can get the basic sidewinder back for free but once you leave or start upgrading you'll need to keep that rebuy cost in mind. Never fly without a rebuy.
Agony Aunt, you're right a full read thru of the manual should be done by every new player and I am working my way through it but as I'm sure you realize, it's hard to do that with the eagerness to get into the game.
The insurance is 5% of the total cost of your ship and upgrades/modules etc. Any crew you have (regardless of they money they've been paid) do not ever buy remlock suits and so, are never revived should they be on your ship should it get blown up.

For reference, you can see the full rebuy cost in the right panel, just under your balance. You always want your balance to be higher than that number, but, since you can get a loan and that number includes the modules, you usually will be able to at least get the ship itself back even if it isn't.
There are a few situations in which the consequences can be a little worse. If you get a large bounty, you might have to pay that off, more so for ganking players. And when you die you drop anything you were carrying, ranging from cargo, which usually isn't a problem, to mission objectives, which can hurt a little, to months worth of scanning data, which... makes up for how low risk exploration is otherwise.
In general, don't buy a ship too far beyond your means and you won't have anything to worry about.
