Before you start the game, make sure the TrackIR program is already running.
I never thought that this would be the problem..
Track IR should work pretty much out of the box? I occasionally use a homebrew IR tracker, the only thing I can think might be causing a problem is the headlook mode. I think it's defaulted to off, and you need to either toggle it on or set it to default on (it's been years since I set it up, might even be mixing it up with another game).
It's a bit late now, but I'll see if I can dig out my tracker tomorrow and test it out for you. Thread might get locked by the mods for being old. If it does, start your own new thread or drop us a message on my profile and I'll see if I can help there.

3x IR LEDs on amazon.
Resistor(s) - how many and their rating depends on your LEDs and chosen power source.
Couple of bits of wire.
Power source (AA or AAA battery holder, or a USB cable, if you don't mind plugging it in).
Something to make a frame. Doesn't have to be particularly sturdy, thick single core wire (a bit smaller than a wire coathanger preferably so it's easier to work with), lolly sticks, whatever you have lying around...
Old PS3 eye cam (preferably with the IR filter removed, but it's not essential). Quite a few other webcams work too. Ideally you want at least 640x480 res, 30fps and a weak (or no) IR filter.
My first prototype used an old bank card, some cable ties, and liberal application of hot glue to make the frame. Had 2x AA battery holders, which could be used individually (handy for swapping out old batteries for fresh on the fly - not that I ever really needed to), or together for longer sessions.
Grab a couple of bits of free software... I can't remember if I used opentrack, freetrack, or facetrack with complete certainty, but I think it was Freetrack - I'll have a look in a mo, I've probably still got it installed. EDIT: OPENTRACK is what I use.

My joysticks on the other hand... Yeah you've got me on that one. They weren't stupidly expensive, but they weren't cheap either.
