Claim throne is impossible because character died.
as Castile i had the opportunity to claim throne of portugal, but the heir died during the war and now the claim throne is just greyd out.
why cant i then just claim the country with some extra unrest or something?
these scenarios really need to be fixed in some way.

You can claim the country, its called annexation. With "Claim Throne" you are basically just putting someone next in line to their throne, which would then lead to a personal union which would need so many years to integrate and so many years to make them useful for war. You can do that or simply annex them within 10-15 years.
you cant seem to claim the country in the war that you just won.
all you can do is take some land, like it was a regular war.

if the character dies, there should be something that informs you that the claim throne is a lost cause, that you need to go do something else.

Of course you cant claim the whole country. Thats not a thing in the game. And why would you? Portugal still has a ruler, i assume, and you went in to aggressively push your heir into his heir spot. Your heir died and you didnt have anything to push anymore, so it failed.

I mean, the game apparently tells you that its a lost cause when the option is greyed out.

But you learned something now. Claim Throne is pointless, even when it works. Go for annexation wars. If you want to roleplay though, Iberia is getting some content in 1.4 where you can personal union the ♥♥♥♥ out of the nations there.
