Only true "bad" vs. "good" choice is whether you kill people after you have defeated them or not. If you drain people of water, you get serum (equivalent of money) and loss of karma.
Not sure if there are quests which give you negative karma.
Its not that simple actually. Its not so much "Karma" but your companions will treat how you act differntly and their choices during cutscenes as well as your own will change as you develop your characters "personality" by your choices. As far as I know there is no Good Guy, Bad Guy playthrough its just choices.

Nope, there is Karma. Look at the right side, there is -1 Karma loss from killing someone.
That does not mean that is all to the game, though, because it still matters more what factions you side for and whether you support your companions or not.

This is not true. Taking Serum from defeated enemies means killing them in a brutal way. Some compaions will leave you over this while others may encourage this while still others may lean either way.
