Pilgrims of the Dark Bug? (Tier 3 reward - Climax hex)

I completed all 3 of the dungeons for this covenant and got the "Selfless Giver" achievement for reaching rank 3 in a covenant but I did not get the "Climax" hex. The first dungeon I did (Castle one) gave me the first tier reward (the first hex) then I did my second dungeon (Shaded Ruins) and spoke to the covenant leader and no reward or rank up message! I then did my third dungeon and Darklurker boss (Black Gulch dungeon) and spoke to him getting the second tier reward hex and the rank up message as well as the "Selfless Giver" achievement. Is this a bug? Makes going for all hexes a pain. Any ideas?

Go back to the place you finished the pact talk to the old man again and he will give the climax
