There are a handful of hidden '?' areas (Not viewable from any sync point) off in these hard to reach areas, but they are more like easter eggs than additional content. A hidden monkey shrine, an all-female bandit camp, the island of a thousand cats, etc.,
I recommend skipping most and just doing mq.
This might sound like a chore (and it is), but on the bright side you'll meet plenty of ronin who serve as money pinatas, discover easter eggs, stumble upon Jizo shrines for the quest to honor 69 (nice!) shrines.
And last but not least, there are plenty of "keys" scattered across the map, that let you get otherwise paid gear though the exchange.
That's what I've been doing since release.
100% cleared the fog of war in five provinces. Izumi Settsu, Yamato, Iga, Yamashiro, and Omi.
Only Wakasa, Tamba, Harima and Kii remain.
As you can see by my hours, it's not as difficult as you may think. The fog of war distance that is cleared around you is quite large compared to other games.
I start by doing the perimeter of a province's borders, then work my way inward. Don't do it in the winter or the snow will slow you down. You tend to run into enemy camps out there which are easy to clear for resources. Otherwise, there isn't much on the outskirts.
Exploring everywhere will land you a lot of Ubisoft keys, animal paintings, base-building loot, and people to kill as you explore inward. I'm sure that I've found more Ubisoft keys than anyone else.
Looks like a 250-300 hour game the way that I play.
116 hours and counting.
