Also does barry have to kill plant 42, because the side door in that room is red and can't open it anymore. Also if I missed a document the room would be red still, correct?

I got it as Chris. Just check the door shoud be OK. I don't know jammed doors, but some show a x after I check both side. Some can only be open from one side so you need to open it.
You must read every document or the room is still red.

The bottom left it says its x'd but another red door in it, did I mess up?

Does every door need to be blue?

No, I think it is OK, the door is locked after you enter the room. It is a cut scene when Barry saves you so I don't think they leave a loophole.

Don't forget you need the closet key for the changing room also don't forget to check Jill's/Chris's prison cell in the end, there's ammo in it

Already went in the closet room, and yeah I see tons of people make mistake by missing the prison. Thanks!

You do not have to visit the closet room, the achievement can be earned on your very first run.

There are no "don'ts". You have to pick up every item from all room that can be picked up, this is it.
