Turbocharger diameter
What are the turbo diameters referring to? The inducer or exducer? Im tryin to use real turbo measurements, for example the TD04HL-15G, which had an inducer diameter of 43.4mm and an exducer of 56mm. which will make a big difference on performace.

Yes the compressor wheel size - the intake of the inducer. The turbine size refers to the exhaust pipe side of the turbine wheel.
In that instance you would set your compressor size to the inducer and turbine size to exducer. It will obviously be a restriction on alot of engines but that is as life like as they have gone so far.

Both the intake (compressor) and exhaust (turbine) wheels have their own inducer and exducer diameters independent of each other.

I think the units you see in-game is the average of the 2 sizes for each side of the turbo, example; 43.4+56= 99.4 then 99.4/2= 49.7 which closest to that in game would be 50mm. At least that's how I do it.
