That's the Mastercrafted Ursine Armor.

Yeah, the Ursine (Bear) armour, after the upgrades. Sadly not as good as the concept art for it, but still decent looking. Pick up the maps from vendors to trigger the witcher gear treasure hunts (Cat, Griffin and Ursine (Bear) ). More info on specific locations is a Google search away.

with my skill build it's literary the best armor in the game (hibrid fight/alchimy)

PPl allways aim for a fight build i'm on something really effective (especially for hard difficulty)
with 25 point in fight (strong and light attack with dodge, and the rest on oil and potions) potion are very use full on hard diffficulty because they can heal you (swallow and white rafar) and for a ice on the cake i chosen fragmentation grenades.
