I have experimented an bit and the following setup gives (in my opinion) an good compromise between image quality and frame rate. I never had crashes or frame drops. Even in city's the number of frames stays well in high limits. I have some pop-ins after an fast travel between zones or start of the game (when all is loading in) but it stays within very acceptable range.
First of all - I have the GOG version, and I do NOT use the Galaxy Client, because it is in beta state and can cause strange things like frame drops and crashes. I also advise to disable any overlay.
For the Nvidia drivers
I have disabled the Nvidia Streamer service for Shield. This one is only needed if you actually use shield streaming, but most people don't need that service. It can cause frame drops and other strange stuff, so it is best to disable this service.
In the Nvidia Control Panel "Manage 3d Settings" I use the following "overrides":
Maximum pre-rendered frames - 1
Multi-display/mixed-GPU acceleration - Single Display Performance Mode
Power Management Mode - Prefer maximum performance
In-Game settings:
Postprocessing (this are personal preferences - I don't like blur and sharpening):
Motion Blur - Off
Blur - Off
Anti-aliasing - On
Bloom - On
Sharpening - Off
Ambient Occlusion - HBAO+
Depth of Field - Off
Chromatic Aberration - Off

Maximum Frames per Second - 60 or maximum (maximum can cause tearing).

Water Quality - Ultra
Grass Density - High
Texture Quality - Ultra
Foliage Visability Range - High

Did you test the difference between "single- and multi monitor performance" because every developer and nvidia always says you should use multi monitor performance mode cos even on 1 monitor it maxes the performance better.
The shild stream service, is it always installed with the driver? I doubt I have it installed, gotta check this evening if it's running.
I will also try starting the game from the explorer and not gog client.
I have one more tip:
When in game alt+tab out and start the task-manager then set priority to high for thewitcher3 exe and go back into game, that improves things too.
No - I have (not yet) tested the difference between "single- and multi monitor performance". I just use this as standard in most games, but I will give it an try...
The Shield streaming service is installed if you use GeForce Experience. Sadly - If you use that program it re-enables that service every time it does an update (graphic drivers or itself). It is annoying, but disabling only takes an minute, so not too bad.
I made an shortcut to the .exe file on my desktop, so I do not have to use the client. If I want to add an DLC or do an update, I download it and install the .exe file. That way I can control when I want to update.
I will check out if the stream service is running on my pc.
The GOG client is really bad atm. I can't recommend it at all, only good thing you can download your games with a click, that's better then the browser version.
