You missed quests, key dialogue choices or just picked the wrong answers.
Effectively you're punished for making choices that encourage your spawn to be more cautious and responsible, instead of reckless and impulsive. Not sure what CDPR was thinking here, especially given Ciri's history when it comes to reckless impulsive decisions and how it would make a lot more sense for Geralt to try to be protective of her.
like:
- if you play snowball with her or not.
- if you destroy avalachs laboratory or not.
- if you go with her to the witch-meeting in the rosemary and thyme, or let her go alone.
- if you take the money from her father or not.
Myself dont really need to do that much backtracking, because i make eversthing else first, and the main story later.

Basically you were a bad father figure to her so it is very possible that when she did what she had to do she decided that there is nothing holding her there so she opened a tear to some other universe. Who knows, maybe she finds more attractive some city in the sky or underwater, some station in space or neon lights of the Night City.
I mean the Red Baron is there to show booze is bad and you chose to drink booze with Ciri?
