Btw i remember i killed Lycan there in release version but 2nd time few weeks ago i didnt see him, did they change it so he appears at night? Anyway he doesnt linked to this monster den.

@OnlyOffensive i did the same thing but i did it exactly reversed than what you did i entered from the swamp killed all the nekkers entered the room in the right grabbed the note by the jarl telling the alchemist and his men to continue their work and then continued into the cave following the blood saw the body of the alchemist looted it for the notes about the situation and a key for the locked chest on the left of the hall after entering the cave got the treasures, realize that there is another path to the cave climbed the slope cleared that cave grabbed all the things i can loot then exited to where you entered unlocking another cave entrance then realizing that the monster den icon was still white so i decided to rest till midnight then the swamp cave entrance a werefolf appeared killed that then nothing i've rested multiple times midnight and tried running the cave repeatedly nothing i really think this is a bug if you enter through the swamp side of the cave i didn't even notice any nekkers wearing a shirt just like the alchemist and the jarl was talking about in their letters nothing... so yeah i really think i just happened on a bug hopefully not but if it is then one more thing keeping me up at night... T_T

In said cave there is a chest. http://steamcommunity.com/sharedfiles/filedetails/?id=675794292 That chest has your standard loot and all but it's abit off to the side and easily missable if you don't go down the right path towards it. The chest though is abit glitchy though but it's still functional.

whenever i load up the game and start to peek into the corner where it is located when i am in witcher sense mode it will still continue to glow with orange aura which we know indicates that there is something interesting about it meaning it's lootable but i've already looted it... which i think is the reason why the monster den is still white instead of grey, i fell like my specific save hit a glitch in the coding where instead of marking the chest as looted somewhow it is still marked as not looted (although i looted it already) thus making it continue to glow with orange aura and making the monster den still marked as uncleared... yeah i reall feel it's an isolated glitch i encountered maybe on how i approached the cave or something cause i entered it through the swamp instead of the cave north east, i really don't know... maybe i will find out on my next playthrough, i will remember this cave and i will do it right next time... :D thanks for entertaining my query i just can't get it off my head kinda feels like i have something stuck on my teeth that i can't remove...
In short, you have to explore the place at night which will spawn a werewolf in front of the cave.
At first, I explored the cave as the OP did, and it didn't grayed out. At this point I found this thread. I tried killing the werewolf but it did nothing.
So I read the comment by mirg, reload the game and enter the cave form the mountain side, not the Caer dhu side. Also, while I was exploring the werewolf came to my mind so I meditated till night. But it didn't grayed out .
And then I tried once again from the Caer dhu side, but this time I meditated before I went near the place. So I killed the werewolf, nekkers inside and loot the key and the chest. When I looted the chest it finally grayed out.

thanks i will remember and try it once i get there again i'm on my first NG+ so once i reach skellige again i will remember this.
Not sure if the time of day changes anything, but I do think you need to enter the cave from the western side. I initially found it by entering from the eastern mountain entrance, which didn't grey out the marker. Upon finding this thread, I tried it again, at night, from the other side. There was no werewolf there for me though, just a cockatrice nearby, which I killed. I then entered the cave and made sure to check the locked chest just to get the "locked. key required" message, hoping that maybe that triggered something in the code that would mark the dungeon as cleared in the end. After killing all the nekkers, reading both letters, getting the key and looting the chest, the marker greyed out. I didn't have to enter the upper part of the cave (the mineshaft on the other side of the illusion) at all. I'm not sure, but I think there's a fair chance that checking the chest while you don't have the key yet is the important detail here. Or maybe it's looting and reading the letters in the right order, who knows. Either way, entering through the western entrance (at night?), checking the chest, looting and reading the letter from the dead mercenary, then looting the letter and the key from the other guy and finally looting the chest should definitely work.
how can a game be this glitchy after 2 years of patching?
Not the first time I had to load a much older savegame and lose a good chunk of time because I did a unmarked or marked quest in the 'wrong' order/way and it became uncompletable.
I swear, this is a huge multi million dollar project and ♥♥♥♥ like this haunts the game years after release
it seems like half the development team is straight up incompetent coders/designers that shouldn't operate on this level but the rest of the dev team can't bear firing "their friends" even if they aren't up to a project of this magnitude.
since the GOTY patch i actually stopped encountering bugged icons... since it already disppears completely after completing the missions, but some bugged merchants are still on like the blacksmith in kaer trolde can't be talked to after going away for a while and a merchant in the crossing from velen to redania, also sometimes after checking the shop and returning to the conversation selections the whole texts disappears all of them and you can only navigate using the arrow keys so you'll just have to check which conversation you actually picked.
i actually still encounter bug on the journal like on the quest for example redania's most wanted were you can choose to give radovid the agate stone or to triss or yeneffer... i always opt to give it to triss cause she is the closest sorceress i can find... but after completing the mission the journal marks it as if i didn't give it to triss and later down the story radovid acts as if i gave it to him.... breaks the immersion because of that bug and kinda annoys me whenever i check the quest journal... i'm like that's not what i did...
