At some point in the books Geralt couldn't find any contracts to kill a monster, so he had to settle in and work in a local butchery of Blaviken. He wasn't really good at it, got many scars from this job, like the one on his eye when the cleaver slipped out of his hand. You can also notice huge chewing scars on his shoulders - a cow bit him.

Blaviken is a village. Geralt was hanging out there when a disowned princess showed up with her deathsquad to kill a mage.

First Geralt got in her pants and then the day after had to kill them all. The lesser evil so to speak as they were intending to massacre the entire village to draw the mage out of his tower.

It was named that because Geralt cuts through them like a hot knife through butter.

Anyway, it does come from something else, a particular scene in which Geralt killed some bandits in the books. It will be one of the things they will include in The Witcher movie, which will be relased later this year, most likely.
