I still fail sometimes because of my own will.
I see no reason in losing health when attacking too long once Dettlaff is able to get hit (stage 2).
Sometimes i can also not see well enough he is starting is raven attack because it is happening off-screen for some reason.
I also two times got hit by the raven-attack because while running away from it Geralt decided to lock on to the target.
So... its just dumb and clumsy designed. Witcher gameplay is not suited for this type of boss fights.

Just use Quen all the time and roll like a puppy. Hit the hearts. Done.

Detlaff isn't that bad. the first time I fought him on 'story and sword' difficulty, he beat me loads of times...
...now he doesn't scare me a bit, even on deathmarch and because of that, he's quite easy. whatever your difficulty level, I'd advise the same tactics.
first off, fight him as you would with any vampire: best vampire oil, dodge his attacks and strike. when you hit him enough he turns into the winged version. This shouldn't take too long.
After that, watch him!
That's the key.
I can't stress this enough! Watch him! If you do, however awkward it is making Geralt roll about and use the camera, if you watch him and time what he does, he's predictable and so you can anticipate his moves.
Keep an eye on what he's doing! He launches aerial attacks with crows or some-such and then he dives. The aerial attacks knock out any quen shield you may have.
Try to dodge both of these attacks by rolling or diving.
The dives can damage you. When he hits the ground, he's stuck for a time...dive in and get a couple of fast attacks, or one strong attack and roll away.
Roll to avoid his aerial attacks, whether you're using quen or not.
Just be aware that after he gets stuck on the ground he leaves red glowing spikes where he was...don't go near these spikes! you get trapped and hit!
Repeat the dodging/rolling/striking until he goes to his next phase.
Then you get dragged to some other world, for want of a better word. Ignore Detlaff at this point! Use quen, if you like, but you don't need to.Roll towards the 4 pulsating balls around the edge of the dome, one after another. They are each marked 'Detlaff'. If you use quen, you can get in another hit or two at each point before you have to roll away...this saves time.
At each one, hit it as many times as you can before the 'real' Detlaff gets to you.
Roll away and do it agin until each of these 4 balls have no health bar.
I guess that it's the last bit you're referring to as the 3rd stage. Actually, it's the easiest, once you get it out of your head that you don't have to attack the hideous red blob that is Detlaff!
roll away from him, hit the four sacs and win!
cut-scene...you've won.
It sounds easy but it takes a bit of practice.

P.S. This is based on an attack or mixed attack/signs build. I've never done it on a pure 'signs' build so that may alter. I'd say that the procedure is the same but hitting him with igni, rather than the sword...others may advise you better if you're using a 'sign' build. Dodging him and his style of attack will be the same, though.

P.P.S. yes, the witcher 3 fighting style leaves a bit to be desired, but if you've played the whole game through as far as 'Blood and wine', I'd have thought you'd have come to terms with its inadequacies by now?
