Sometimes I fight the lot of them, including dismounting half way up the hill to have a go at the ones in the side street to the right.
At the top of the hill, I sometimes get it right, sometimes not. What seems to work for me is to use Ciri's ability to 'fast travel' past them and open the door behind them. It's hit and miss though. If they are close enough to Ciri the option to open the door isn't there.
Likewise, getting on the horse in the first place can be problematical. Ignoring the Redanian guards and going straight for the horse is best, however much you'd like to wipe out Whoreson Junior's thugs. If not, you're stuck fighting them off while your mount rears and whinneys.
I don't think this quest was well done, but I hope what I've said works for you...you have my empathy!
Also I agree with you about Ciri's Walmart sword! I'm from the UK, so our equivalent would be a sword from Lidl...probably a plastic one that you'd buy for your child.
Only time I died was to crossbow bandits but I think there's just one or two of them for bandit group so it's easy to take them out first.
Good so that's what I did then blink to archers cut into them and rinse and repeat until they're all dead. Ciri just has to keep blinking every two secs or so during lure because arrows can come even from outside screen area.
I´m afraid that I have to start a new game because I finished all side quests and Witcher Jobs in Velen and Wihite Orchard. It would suck to do all of them again.
Disable mods if you have them, they may affect. Try loading a save before the quest started.
Junior lackeys lvl doesn't matter, Ciri doesn't has level nor penalizers due to lvl difference, what matters if if they take damage when they get hit.
The door only opens when all the lackeys in front of the door are dead, usually if doesn't open is because there is some lackey alive somewhere. It could happened that a lackey fall below the ground(in any instance an enemy can fall below ground level due to a bad collision detection) or just some lackey may have falled from the clift to the left. Retrying it should solve it providing another lackey doesn't go out of your reach alive.
