Your paying them monthly to play the game period. Thats additional money for what amounts to server maintance on there end. They use that additional money to add content to the game to keep you playing, thus keep you paying. Thinking they are giving you "free" content is missguided at best.
The real irony is that if betheda had released horse armor now with a new released game (instead of when they did) it would not have anywhere near the social impact it did.
That is what people want, extra hair styles for example, should NEVER cost extra, while major new additions would end up in an expansion.

While this was true with Witcher 3.
The majority of dlc sold does not fall under this category. Ubisoft for instance plans out all of there dlc before the game even launches.
