You can only enforce a "promise to protect" by military powers. That's why you have the opportunity to declare war with less Grievances. And you get Grievances towards AI, when they attack your City-States.
There is an alternative, however, if you are willing to devote the units necessary. If you surround the city hex of each of your city states with units, then no one will be able to capture the city state without first killing at least one of your units. This will make your city states effectively immune to aggression by your friends and allies.
Except when the city state is on the coast. They roled up a sub and keep lobbing missles. Its now 120 turns in and they cant take the citi state yet.

Also you can upgrade those Levied units to your current designs so that when the City State gets them back, they will be a lot stronger to defend themselves.
Protectorate War
This Casus Belli is unlocked with Diplomatic Service. It is used to declare war on a power that has attacked one of your allied city-states. Note that this Casus Belli remains active only while the power and the City-State are at war; if they make Peace, or if the City-State gets conquered, the Casus Belli vanishes. As is the case with Liberation and Reconquest Wars, other leaders understand and admire this kind of loyalty, and no warmonger penalties apply for either declaring the war or liberating the city-state. Again, you can use this if another force is becoming too powerful and is beginning to take down your neighbors
City-State Emergency: A civilization has captured and is occupying a city-state. The Members must liberate the city-state. All Members will go to War with the Target and have Peace, Open Borders, and Shared Visibility with each other. If the Members liberate the city-state, then they receive Civ6Gold Gold per turn for each of their Envoy6 Envoys there. If the Target retains control of the city-state, then he wins, and receives bonus Civ6Gold Gold per turn for each of his TradeRoute6 Trade Routes there.
Also, I didn't say you could protect the city state from taking damage, I said you could block attempts to capture the city. They will naturally reduce the city with their ranged attacks, but they won't be able to capture the city if they can't move a unit adjacent to it.
Which is why even when playing a pacifist run, declareing friendship or alliances is a very serious thing to do.
City State litterally has its attack strength listed next to its name.
