if it asks for wan or lan port, it's still 27960 UDP
There is no other port, and the protocol is UDP.
If you open both TCP and UDP, the TCP won't be needed, but it would work as well.
If it asks for a "service type" or application, unless your router has Quake Live (or Quake 3) already in the list, you are better of creating a custom one, and naming it Quake Live.
Some routers ask for a "name", just for the sake of organization, it's not used at the technical level, just put "quakelive".
The internal host or LAN IP, is the IP if your computer (locally) you can by opening a command prompt (search cmd or command in your start menu). And typing ipconfig. Look for a line that says: IPv4 Address.
Strongly recommend finding a generic guide for your router on how to open ports. The sections and settings are named differently per router. http://portforward.com/
Typical names, of the settings you are looking for:
* Forward Rules
* Port Mapping
* Port Forwarding (port triggering is something else)
* Virtual Server
Going from Router A to Router B, and then to the local ip of your computer.
