Make sure to match the resolution of your desktop or r_mode 23 can cause this since in quake iii arena its 4K, that is the limit of you being able to get into the game fullscreen without OPENGL joining the blue team. I propose the following: once inside the game you may experiment with web_zoom and factor it to downscale the 1/4 screen with web_zoom 50. (Another example of this is using mode 17 and put web_zoom to 125. This will bring 5:4. Smaller resolutions are matched with above 100 web_zoom. High resolution with web_zoom beneath 100. Magnification is the distance the viewer is to the native desktop resolution viewed through 1920 x 1080 window size in quake live while your video card remains on the same signal resolution.
Playing with the web_zoom in relation to different mode and make it correct while you play on 4 K signal resolution is where r_mode 1 at 144 p becomes 1440 p by 1920 x 1080 p with web_zoom 200, bringing it back to 4 K thus matching your cards output signal resolution.
