Do I take it you're playing with Kristoff and Iskra? Kristoff is just superhuman. ALWAYS leave the scavenging area before time is up but that won't affect the tiredness gained overnight.
Possible exception the scenario with Misha, which I suspect is bugged. If you clear the house on the first day, you find 2 bottles of pills. If Misha is the one sick, and an adult gives a bottle to him on the first day, one of the remaining adults in the house that night automatically administers the SECOND bottle during the night. So it seems either (1) if giving Misha the first bottle on the first day, don't pick up the second bottle, leave it in the cupboard so it doesn't get auto-administered during the night, or (2) don't give Misha any pills on day 1, leave a house-member to auto-administer a bottle during the night so that only one bottle gets used.
(note whether Misha gets one or two bottles on the first day/night makes no difference to their outcome, they generally go to slightly sick on day 2)
I did this in a recent run-through (option (2) - not giving Misha pills on day 1) and the pills did indeed get auto-administered during the night, so saving a bottle (if having cleared the house and picking up two from the cupboards on day 1). Can see the auto-administer having happened at 11.00:

In the Misha scenario, only one of the house members start severely ill. If its Misha, then none of the others are sick. If you pick up the two bottles of pills and give one to Misha on day 1, the second bottle gets auto-administered to Misha during the night (I only tested this twice and both occasions that's what happened - maybe it doesn't always happen, but the two times I tried it, it did).
If not giving Misha pills on day 1 and a bottle being in inventory, then it gets auto-adminstered during the night and Misha starts day 2 slightly ill - so giving the first bottle of pills during the day has no effect other than to use the additional pills as in both scenarios Misha starts the next day slightly ill. Only difference is 1 bottle of pills being used vs 2.
There are no severely ill breadwinners in play on night 1, as only one house member is severely ill, which is Misha (if one of the other members is severely ill, then Misha is not ill on day 1 as only one house member starts severely ill in this scenario).
So not sure who is going to steal any meds. If you mean from the night 1 raid, then 2 guards can stop that as 2 guards stops any loss on night 1 (at least I never saw any loss on night 1 when posting 2 guards).
