Trade your excess medical supplies to Matey at the Garage. Matey has a lot of good stuff to trade, and you should see a morale boost following. The Church is also a pretty good place to go.
The other reason, I am guessing here, is due to the fact that you donate it as a trade. So regardless of whether you actually take something from the doctor, you're still trading and not just giving it away like when a neighbor comes to ask for stuff. One of my survivors reacted to me donating to the hospital with something along the lines of that it was a good thing we didn't focus on our profit when we traded with the doctor.
The hospital is only good for healing injured/sick survivors in my opinion. Also, if you're willing to steal.
It never shows up in my endings.
I just did it few hours ago because i was curious with alternative ending. I came to hospital once and give some of my meds.
I can the characters got morale boost and their story. Also showed up when the game ends.
