The new system is akward, you need a weapon to lean, you can't do it bare handed, and since you need to hold right click Indiana get into his weapon throw stance, I really don't get why they made the leaning system so clunky
It does occasionally work without holding anything for some reason but it's too unreliable to actually use else you just end up winding up a massive punch whilst creeping around.
No, you don't. Holding the left trigger is like a 'charge up' button, so when it's pressed while holding nothing, you can then lean. If you want to punch with your left hand, you only need to quickly tap the trigger.
I can also only lean when holding something as well (keyboard/mouse plus PC). The game says in the in-game manuel you'd only need to hold the right mouse button and then tap the lean buttons. But Indy won't do it unless you're holding something. Same as claimed in this thread. And this article.

This is some of the weirdest designs ever. Thief got it right 25 years ago -- just two simple buttons, lean left/lean right, that's it.
Then you press LT to "aim", LS / RS to lean left / right.
If you press LT with empty hands you "charge up" and pressing LS / RS using actions but do not lean.
