It also helps to collect all the other things, because sometimes you find jounal entries that bring up further mysteries or spots of interest.
I cleared everything else in the game, journal achievement is the last one I need for 100%. The counters always were a bit wonky and those in Travel tab still say I’m missing field work in a few levels. The journal note counters there, however, say I got everything iirc - will double check once I’m back home. I did make sure to save before revisiting other areas though.

I had a similar case with relics until collecting the last one in game but for journals I have no markers left :(
I found a workaround for people struggling to unlock the achievement (taking a hint picture of some puzzles like the Fountain of Confession) but it didn't work for me (neither did it increase the counter in Chapter Notes). I'm going to try with other puzzles though as it seemed to work for other people for whom the fountain picture didn't help.
EDIT:
Turns out I'm stoopid. I assumed the only notes required are the Field Notes because that's what was shown under the Discoveries section but I'm actually missing some quest-related notes I haven't picked up while progressing through the game. I picked up one and the Vatican counter for Chapter notes went up.
Although I still have some ghost markers leading to empty spaces so I had doubts it will actually work.
