The icon would still be shown if you still had that specific mission objective selected.
It's annoying you can't look at other maps without changing the objective.
i've been to all the accessible underground areas, it's always off map.
I know it’s underground, I’ve been everywhere under the fountain and the excavation site, nowhere else close enough to get close

mapgenie says there are no underground notes there.

yes, it appears in both "the mad priest" and "a nun in trouble" fieldworks. but the missing note is a field note (57/58) i have all the notes for both fieldworks.
Where the icon is on the map is nothing. Not even sure what it is pointing at. Right below it is nothing but void, and the "Necropolis".
i got the achieve for all journal notes after taking the photo. i read another post about it. i still have 57/58 field notes and 1 missing. but there is nothing actually missing.
finished the game. started another playthrough on harder difficulty. still have that save slot though
Yea, that's why I said you need to have the objective with the note highlighted on the whole time and travel directly to the underground area. When you get there, your map should swap to the underground map while you still have the objective with the highlighted note on. If there is no note in that underground area, the map shouldn't have the icon and you move on to the next underground section.
